I. Osmotic diuretic effect
1.Increase plasma osmolality:
Mannitol is a hypertonic substance that rapidly increases plasma osmolality after intravenous injection.
As there is an osmotic pressure gradient between plasma and renal tubular fluid, this prompts the transfer of water from renal tubular fluid to plasma, thus reducing tubular reabsorption of primary urine.
2. Increases blood volume and renal blood flow:
Mannitol increases blood volume, which in turn increases renal blood flow.
This helps to increase the glomerular filtration rate, allowing more water and solutes to be filtered out of the glomeruli.
3. Inhibits tubular reabsorption of water and electrolytes:
In all segments of the renal tubule, mannitol inhibits the reabsorption of water and electrolytes such as sodium ions through its hypertonic effect.
Especially in the thick segment of the ascending branch of Henry's collaterals and the proximal tubule, the effect of mannitol is more pronounced, resulting in a powerful diuretic effect.
II. Tissue dehydration effects
1. Reduce cerebrospinal fluid production:
When mannitol enters the cerebrospinal fluid through the blood-brain barrier, it increases the osmotic pressure of the cerebrospinal fluid.
This allows the water in the cerebrospinal fluid to be transferred to the blood, thus reducing the amount of cerebrospinal fluid generated and lowering intracranial pressure.
2. Reduces tissue edema:
For localized tissue edema, such as cerebral edema and pulmonary edema, mannitol induces the transfer of water from the tissue interstitium to the blood vessels by increasing the osmolality of local tissues.
This reduces tissue edema and improves blood circulation and the function of local tissues.
